AGE DECLARATION AFFIDAVIT

Declaration of Age Affidavit Format for Minor Female.

Age Declaration Affidavit is used to validate the Date of Birth of an individual who cannot present official Certificate of Birth. 

For individuals under the age of 18, another person, preferably a blood-relative, who has personal knowledge of the minor will fill out the Affidavit. 

The following information is required to prepare Age Declaration Affidavit for minor.

Minor Birth Details
Passport Photo, Day, Month and Year of Birth, Place of Birth .
