LOSS OF SIM CARD AND SIM PACK AFFIDAVIT

Loss of SIM Card / SIM Pack Affidavit Format.

If you have lost your SIM Card, create Affidavit of Loss of SIM Card to start the retrieval process. You may be required to present a valid means of identification.  

The following information is required to prepare Loss of SIM Card Affidavit.

Deponent Contact Info
Email address, Phone number.

Deponent Details
Passport photograph, Full Names,  Gender, Religion, Nationality, Street Address and State,

Lost SIM Details
SIM Network, SIM Number
